That 36 inch bar on my 372XP did the same thing last winter on a big oak limb. First thing, check your chain tension. A lot of guys run them too loose and that's usually the culprit. If the tension is good, look at the chain brake band and the clutch drum - if either is worn down, the chain can slap around and come off. Also, make sure your sprocket tip isn't worn out, that's a hidden one that'll throw a chain every time. I had to replace my whole bar and chain setup after that happened once and it was a pain.
