Rear quarter panel rust came back after 8 months

Fixed a rust spot on a Tacoma rear quarter back in March. Ground it out, welded in a patch, sealed it proper. Customer just came back last Tuesday with bubbles showing through the paint already. Pulled the paint off and found the rust creeping from behind the seam I thought I got clean. Turns out the inner panel was worse than the outer and I missed it on the initial check. Spent the afternoon cutting out twice the metal and welding in a bigger section this time. I'm charging them parts and labor only, not my markup on the first repair. Anyone else have rust sneak back after a patch job like this?
