Can we talk about who is responsible for flagging the load on a tight pick?

I was on a job in Charlotte where an operator got blamed for a wobble on a 12-ton pick over a pool, but the rigger had signaled him through without checking the radius, so whose call was it really.

Operator's job to check radius before anyone flags anything. His fault.

Hell yeah, I watched a guy swing a load right into a parked truck because he wasn't paying attention.
