Thought my panel schedule was fine, then the arc flash calc said otherwise

I ran the numbers on a 400 amp service at a church in Fresno last week after I changed out a feeder breaker. AFC levels came back way higher than I guessed, like 18kA on a panel I assumed was safe. We had to add series rated breakers and relabel everything before the inspector would sign off. Anyone else ever get blindsided by the calc after a simple swap?
